There are 2 steps to return to work:
COVID-19 Staff Surveillance Tool
Return to Work Tool
The first step if you have symptoms, a positive test, or an exposure is to complete the COVID-19 Staff Surveillance tool. This tool will inform you on the steps you need to take to return to work. Once the steps are completed, staff, physicians and learners need to complete the new Return to Work tool.
When to complete the COVID-19 Staff Surveillance Tool
You should use this self-reporting tool if:
You have one or more typical or atypical COVID symptoms.
You have had contact with someone who is COVID positive or have had an unprotected exposure.
You have returned from international travel.
You are reporting a positive rapid antigen test or a positive PCR COVID test result.

Tip: you will need to copy down the steps carefully at the end of the tool to ensure you have completed all the steps for return to work as quickly as possible. Once all steps have been completed, staff, physicians and learners are to complete the Return to Work Tool (see below). The criteria to receive Return to Work clearance has been expanded to include instances where team members experienced discrete or prolonged exposure.
After completing the COVID-19 Staff Surveillance Tool, you may be directed by the tool to take a COVID-19 test. Please visit the NYGH Team and Family COVID-19 Testing Clinic located in the tent by the ED. No appointment is required. Again, only visit the clinic if you have been directed to do so by the COVID-19 Staff Surveillance Tool.
The NYGH Team and Family Testing Clinic will offer:
Self-serve PCR test(s)
Rapid Antigen Tests (RATs) for pickup, as directed by Occupational Health

If you are positive or have had contact with someone at home who is positive, you are not fit to return to work and must report this through the COVID-19 Staff Surveillance Tool.
Once you have completed the COVID-19 Staff Surveillance Tool by following all required steps, you must complete the Return to Work Tool to notify Occupational Health and obtain clearance for returning to work. Complete the RTW Tool if any of the following apply:
Symptoms have improved and you have a negative PCR test.
After discrete exposure.
After prolonged or continuous exposure.
After testing positive for COVID-19.
